Current Florida license as RN, or current licensure from another state plus a temporary Florida permit. Valid driverâs license and automobile insurance in the State of Florida. Reliable transp! ortation to meet visit schedules. Minimum two (2) years RN experience where physical assessment skills were utilized. Must be an RN graduate from a state approved school of nursing. Bachelorâs degree in nursing preferred. Current (BLS) CPR certification required. Prior experience in oncology, home health, Hospice or palliative care setting is preferred. Must be able to handle sensitive issues appropriately and with sound judgment, maintain professional boundaries, and maintain strict confidentiality. Must be able to effectively plan, organize and prioritize work. Must be self-directed and be able to work well independently in the field. Extensive standing and walking will be required. The ability to lift or move 75 lbs. using appropriate lifting protocol and assists will be required. Must be able to hear, read, write, and speak English clearly.

Definition
As part of the Interdisciplinary Team (IDT), the Admissions RN conducts! the initial admission process for patients referred for hospi! ce services. The Admission nurse initiates the plan of care based on identified immediate needs, and communicates with the IDT.

Shall be familiar with and practice the TCH mission, vision, and values; as well as the eight behavioral objectives.
Key Result Areas
- (50%) Utilizing the nursing process, collects comprehensive data relating to the hospice diagnosis while conducting a nursing assessment based on hospice protocols and guidelines. Identifies the physical, social and emotional needs of the patient and family. Analyzes the assessment data in determining hospice appropriate diagnosis. Complies with state laws and regulations with respect to the hospice protocol and CMS guidelines for reimbursement. Prepares and maintains accurate medical records, charts and documents to support the diagnosis.
- (10%) Maintains a sense of compassion at all times and is skilled in overcoming objections to hospice services.
- (10%) Consul! ts and collaborates with the Chief Medical Officer and other physicians.
- (10%) Performs other duties as directed by the Director of Admissions.
- (10%) Adheres to the practice of confidentially HIPPA guidelines for patients and families.
- (10%) Seeks continuous learning opportunities and professional growth and development.
