CNA Hospice House (PM)

Location: Lakewood Ranch Florida

The CNA is responsible for providing services to meet various care needs of the patient while advocating independence and maintaining functional ability. Respons! ible for documenting patient care appropriately. Performs responsibilities under the direct supervision of a registered nurse to meet the needs of and assist the patient as defined in Policy 9.23 Certified Nursing Assistant Patient Care Activities â€" HH.

Primary tasks, duties, and responsibilities:
1.Provides basic personal care such as bed, shower or tub bath, oral hygiene. Toileting, skin care, ambulation, and exercise as assigned and directed by the RN.
2.Measures and records oral, rectal, and axillaries temperatures, pulse, and respiration, and blood pressure when ordered within the training limitations of the CNA.
3.Assists with household tasks directly essential to patient’s health, ie. cleaning, supply organization, meal preparation and or/assistance.
4.Communicates with and regularly updates the RN regarding current observations and needs of the patient.
5.Prepares meals based upon patient preference and menus as per regulatory! standards.
6.Will perform all duties within CNA scope of ! practice..
7.Collaborates with other service providers to discuss the patient plan of care and frequency of visits.
8.Utilizes universal precautions.
9.Ensures confidentiality of patient records/information.
10.Participates in meetings to enhance team communication, coordination of services, and quality services.
11.Participates in interdisciplinary case conferences and in-service education programs, as required by Tidewell Hospice, and patient needs.
12.Participates in performance improvement activities.
13.Completes documentation accurately and in a timely manner in accordance with the patient’s plan of care and Tidewell Hospice policy.
14.Assists in clerical administrative duties as requested/ assigned.
15.Assists in performance/and completion of Life Safety Inspections.
16.Assists in Hospice House Disaster preparedness and is available for assigned duties throughout the disaster.

Physical Requirements:
1.Heavy Work: Exerting 50 to 100pounds of force occasionally (occasionally: activity or condition exists up to 1/3 of the time), and / 25 to 50 pounds of force frequently (frequently: activity or condition exists from 1/3 to 2/3 of the time) to move objects, including the human body.
2.Standing/ Walking: Must stand/ walk frequently and intermittently to accomplish tasks. May also chose to stand while completing alternate duties.
3.Lifting: Must frequently raise objects including the human body, from a lower to higher position or move objects, including the human body, horizontally from position to position.
4.Pushing/ Pulling: Must occasionally use upper extremities to pus! h and / or pull objects, including the human body, in a sustained motion! .
5.Climbing: Must frequently climb in and out of a car. Must frequently ascend and descend stairs, uneven surfaces, or obstacles, which may be located indoor or outdoors, and may not be handicap-accessible.
6.Stooping / Kneeling / Crouching: Must frequently stoop, kneel, or crouch during the provision of patient care.
7.Balancing: Must maintain balance, which is necessary for ordinary locomotion and body equilibrium.
8.Sitting: Must sit frequently when traveling to patients’ homes and may also choose to sit while completing administrative duties.
9.Reaching: Must frequently reach to administer patient care services, treatments and procedures.
10.Fingering/ Grasping: Must frequently finger and / or grasp objects, including medical supplies / equipment, the human body and a writing instrument, during the provision of patient care.
11.Feeling: Must frequently touch objects, including the human body! , to perceive attributes such as size, shape, temperature, or texture.
12.Talking: Must frequently exchange information in person and on the telephone to patients, doctors, family members, and co-workers by means of the spoken word.
13.Repetitive Motions: Must frequently use upper and / or lower extremities in a repetitive motion, including writing and driving.
14.Sensory Needs: Must posses fine motor skills, visual acuity, (including near and far vision) color discrimination and depth perceptions, and those senses necessary for patient evaluation/ assessment, communication and driving, including sight, touch, hearing and smell.
15.Visual and auditory acuity.
16.Able to lift and turn patients
17.Able to walk long distances.
18.Neat and orderly appearance.
19.Good communication skills
20.Abel to care for all patients regardless of personal circumstances.

Working Conditions:
1.Subject to environme! ntal conditions: activities occur inside and outside.
2.Subject! to a variety of physical conditions within the Hospice House: safety issues may include basic house safety, fire safety, environmental safety, and bathroom safety.
3.Subject to occasional exposure to hazards associated with blood and potentially infectious materials.
4.Subject to occasional exposure to hazardous substance and hazardous waste.

Educational/Professional:
Completion of State approved Certified Nursing Assistant training program and/ or successful passing of a competency evaluation program which meets State and Federal guidelines.

Knowledge, Skills and Abilities Required:
1.Ability to relate to and provide care for patients with acute, chronic and life limiting illness experienced by patients and families.
2.Ability to function in any house situation regardless of age, race, creed, color, sex, disability, or financial condition of the patient.
3.Able to work with a wide variety of people and all levels of ! colleagues, and interact as a member of a multi-disciplinary team
4.Ability to work 12 hour shifts as required to meet identified patient needs.
5.Absolute integrity and the use of good judgment to handle the professional and confidential nature of assignments.
6.Ability to adapt to changing organizational needs affected by internal and external sources.
7.Speaks, writes, read and comprehends English. Bi-lingual preferred.
8.Possesses valid Florida driver’s license.

Machines/Equipment, Tools Used:
Able to safely use equipment needed for patient care such as: Bedside Commode, Bed Pan, Lift, Wheelchair, Electric bed, Walker.
