Job Summary:
The Home Dialysis RN, under the supervision of the Home Dialysis Clinical Coordinator, is responsible for providing and supervising Home Hemodialysis care and treatment, develops nursing care plans involving assessment, implementation, evaluation, training and education; communicates with the physician and other members of the healthcare team to interpret, adjust and complete patient care plans and assists in research studies. The RN is responsible for assisting and supporting the unit management team in achieving quality, patient satisfaction, staff development, business management, employee satisfaction, teamwork and culture. Provides safe, effective delivery of age-specific patient care as overseen by the unit Medical Director in compliance with the standards outlined in the policy/procedure manual, as well as regulation set! forth by the hospital, state and federal agencies. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.

Knowledge Skills, Education & Experience Required:
Demonstrates ability to work with any patient or staff, regardless of race, gender, disease process, lifestyle, religious or cultural beliefs or treatment.

Ability to communicate effectively in English, both verbal and in writing, utilizing all available internal and external methods e.g., wireless, fax, voicemail, email.

Graduate of school of nursing

Minimum 2 years Dialysis Experi! ence (acute or chronic setting)

Job Responsibilities:
Demonstrates through behavior Florida Hospital’s core values of Integrity, Care, Balance, Excellence, Stewardship and Teamwork.

Demonstrates applicable knowledge of policies and procedures related to Dialysis and ability to perform technical skills for the Hemodialysis Registered Professional Nurse including the patient’s treatment, appropriate use of dialysis equipment and water treatment system and emergency procedures, as determined by the competency assessment program.

Develops and assimilates theoretical and technical knowledge with observations, assessment and communication skills to! detect dialysis and non-dialysis related problems. Obtains, writes, transcribes and notes physician’s orders as needed and follows through appropriately. Initiates, monitors and discontinues dialysis treatments according to established procedures. Assesses patients’ pre/post treatment and reports problems/significant information or change in patient condition to the physician and Clinical Coordinator. Establishes priorities of care, both emergent and non-emergent.

Documents clearly, completely and accurately all pertinent information on patient’s chart or electronic record. Participates in the formulation, update and revision of nursing care plans, including long and short term goals, according to established guidelines.

Administers and documents medications as authorized by the physician and as further authorized or limited by the Board of Nursing. Obtains Blood samples and other specimens as ordered.

Recognizes and utilizes opportunities ! for patient and family education and training. Continues evaluation and! provision of individualized quality patient care, maintaining frequent contact with in-home patients by telephone, home visits and doctor’s appointments when applicable. Assesses, evaluates and recommends patients and their assistants (where applicable). Periodically re-evaluates all home patients’ procedures and updates their training when needed, when applicable.

Functions as a role model by utilizing critical thinking skills. Exhibits willingness to try new ideas and keep current with dialysis issues and health technology. Demonstrates effective verbal and non-verbal communication skills in receiving, processing and transmitting information. Assumes administrative duties under the direction of the Home Dialysis Clinical Coordinator. Acts as a resource person for staff within the scope of practice. Assists in orientation and training of new and existing staff.

Monitors Quality Assurance on a periodic basis. Implements or recommend change. Assists in! CQI projects with the healthcare team. Actively participates in unit staff meetings and in-services.

Maintains a safe environment for self, patients and staff by utilizing proper body mechanics and follows universal infection control precautions and cleaning procedures under OSHA regulations. Dialyze isolation patients utilizing isolation precautions and prescribed procedures. Performs or assists in the performance of all emergency operational procedures, including the evacuation of patients in the event of a fire and/or disaster.

Communicates any water, equipment, or facility problem quickly and appropriately following facility guidelines for reporting. Reports all equipment and supply problems appropriately and according to policy to repair personnel/Bio-med support staff. Picks up and delivers supplies, blood samples, records and reports as directed by the Home Dialysis Clinical Coordinator. Performs additional duties and is responsible for specific pr! ojects that are assigned by the Home Dialysis Clinical Coordinator.
