As a Hemophilia Telehealth Nurse you'll be responsible for providing care via education and support to hemophilia and blood clotting disorder patient populations through telecommunications in accordance with Nurse Practice A ct and licensure guidelines. You’ll utilize critical thinking and communication skills to assess, plan, implement, educate and evaluate patient outcomes. The end goal being optimal health outcomes for the patient and encouraging patient compliance to prescribed therapy regimen. The Hemophilia Telehealth Nurse will use excellent written and verbal skills plus advanced computer skills in order to interact with patients and other key partners.

- Use your clinical expertise to conduct telephonic health assessments, provide appropriate education and clinical inventions to patients.
- Develop deep relationships with patients and/or their caregivers functioning as a resource and advocate to enhance quality of care.
- Work autonomously to coordinate patient care and compli ance.

Required Qualifications
- Registered Nurse with current license in state of residence with multi-state practicing pri vileges and the ability to be licensed in all states.
- Minimum 5 years recent clinical experience preferably in a hospital, home care or similar setting.
- Minimum 1 year of hemophilia and blood clotting disorders patient management experience plus IV infusion access
- Experience using Microsoft Office, including Word, Excel and Outlook to document and track patient activity.
- Bilingual Spanish/English highly preferred
- Telehealth experience
- Pediatric care experience
- Home health experience
- Knowledge of disease management, specialty pharmacy or medication adherence experience
- Ability to successfully navigate change, organize workload and meet deadlines
- Ability to work independently while adhering to company guidelines
- RN Diploma, Associates or Bachelors in Nursing
