DUTIES:
This position is for a professional social worker whose duties and responsibilities focus on performing comprehensive and thorough psychosocial evaluations of patient's emotional, social support, and financial resources that may adequately support or not support a successful Liver, Heart, or Lung Transplant. The incumbent is responsible for evaluating prior compliance issues and counseling the candidates about such issues. At the full performance level of GS-11 the incumbent is expected to:
Independently perform case finding functions and outreach, identifyi! ng and screening patients for psychosocial needs. Must possess the know! ledge and ability to independently implement treatment modalities, provide counseling or psychotherapy for individuals, families, and groups.
Independently conduct psychosocial assessments, developing treatment plans in collaboration with the veteran/family and with the interdisciplinary treatment team. Should possess the skills to offer psychosocial interventions and ability to measure outcome of such interventions.
Able to coordinate community-based services, including information and referral for additional services from other VA programs, other government programs and community agency programs.
Experienced in making rapid assessments and developing crisis management plans to maintain the patient in the home, for admission to acute, short-term, and/or long-term placements. Has access to multiple resource directories in addition to previously developed resources to meet the demands of a crisis.
Independently identify high-risk patients ! and provides case management services. Will provide crisis intervention services, seeking to address the cause as well as the presenting complaint, coordinating family conferences and serving as liaison to family members. Has working knowledge of and experience in use of medical and mental health diagnoses, disabilities and treatment procedures - including acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology.
Establish and maintain effective therapeutic relationships with veterans and their families. Able to independently work with patients and families who are experiencing a variety of psychiatric, medical and social problems utilizing individual, group, and family counseling and therapy skills.
Independently provide consultation services to other staff regarding the psychosocial needs of veterans and the impact of psychosocial problems on health care and adherence to treatment plans. ! Ability to evaluate incumbents own practice through professional peer r! eview, case conferences and other organizational means.
Will have a general knowledge of veterans' benefits and services relating to special programs, service connected compensation, and non-service connected pensions. Advanced knowledge of community resources as well as VA programs that will support the psychosocial and financial needs of veterans living at home.
Ability to meet the needs of the customers while supporting VA missions. Consistently communicating and treating customers in a courteous, tactful, and respectful manner. Ability to provide the customer with consistent information according to established policies and procedures.

Grade Determinations.
In addition to the basic requirements for employment, the following criteria must be met when the Professional Standards Board determines the grade.
GS-9 Social Worker
Experience, Education and Licensure.
GS-9 is the entry level! grade for the GS-185 social work series and is used for social workers! with less than one year of post-MSW experience and for social workers who are not yet licensed or certified at the independent practice level. Social workers at the GS-9 level are working toward completion of prerequisites for licensure or certification. In addition, the candidates must demonstrate the KSAs below.
KSAs
Ability to provide psychosocial treatment to a wide variety of individuals from various socio-economic, cultural, ethnic, educational, and other diversified backgrounds. This requires knowledge of human development and behavioral (physical and psychological), and the differential influences of the environment, society, and culture.
Ability to work with patients and families who are experiencing a variety of psychiatric, medical, and social problems utilizing individual, group, and family counseling skills. Work with more complex problems is done under close supervision. With guidance from the social work supervisor, ability to asse! ss the psychosocial functioning and needs of patients and their family members, and to formulate and implement a treatment plan, identifying the patient's problems, strengths, weaknesses, coping skills, and assistance needed.
Basic knowledge of psychosocial treatment modalities and, under supervision, ability to implement treatment modalities in working with individuals, families, and groups to achieve treatment goals. This requires judgment and skill in utilizing supportive, problem solving, or crises intervention techniques.
Ability to establish and maintain effective working relationships with clients, staff, and representatives of community agencies. Ability to communicate effectively, both orally and in writing, with people from varied backgrounds.
Knowledge of medical and mental health diagnoses, disabilities, and treatment procedures. This includes acute, chronic, and traumatic illnesses/injuries; common medications and their effects/side e! ffects; and medical terminology.
Basic skill in the use of com! puter software applications for drafting documents, data management, and tracking. Ability to learn and utilize software programs in use by VHA.
GS-11 Social Worker
Experience, Education, and Licensure.
Promotion to the GS-11 full performance level requires completion of a minimum of 1 year of post-MSW degree experience in the field of health care social work (VA or non-VA experience) and licensure or certification in a state at the independent practice level.
OR
In addition to meeting the basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required 1 year of professional social work experience in a clinical setting. In addition, the candidate must demonstrate the KSAs below.

Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services.

Veteran's Preference
