Additionally, be able to promote an awareness and understanding of positive ethical and moral principles consistent with the mission, vision, and values of Catholic Hospice and those required by law. The Admissions Nurse is a Registered Nurse responsible for evaluating referrals and coordinating patient/family needs for admission to the Hospice Program and coordinating with the Interdisciplinary Team members as required.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
Commitment to Hospice philosophy and mission.

Commitment and Implementation of Catholic Hospice's "Customer Service Guidelines."

Ability to interact with the public and other employees in a positive manner and promote a cooperative working environment.

Ability to coordinate several projects at the same time with competing demands and tight time lines.

Ability to handle sensitive info! rmation and maintain confidentiality.

Computer litera! cy required with proficiency in word processing and general computer applications.

Evaluates assigned referrals for appropriateness for admission and verifies terminal diagnosis and prognosis.

Contacts patient/family within twenty-four (24) hours to set up appointment to explain hospice services.

Communicates to patient’s attending physician, referral source and Catholic Hospice office staff patient’s/family’s decision regarding hospice services.

Performs the patient assessment and physical on admission to determine the current health status of the patient and establishes the written plan of care with the IDT.

Evaluate and insure accuracy in the preparation of documentation.

Demonstrates knowledge of Catholic Hospice, Inc., polices and procedures and pertinent State and Federal Rules and Regulations.

Respect and maximize team effort through interactions with both clinical sand support staff.

Abili! ty to travel to any location throughout South Florida as needed and expand territory as business needs dictate.

RESPONSIBILITIES:
Ensure effective communication with Interdisciplinary Teams.

Obtain written consent from patient, family or patient assignee, when criteria for hospice admission are met.

Participates in providing education to team members regarding nursing aspects of patient/family care on admission to the program.

Provides accurate and appropriate documentation of patient/family services

Adjusts work assignments and schedule in response to changing patient and/or organizational needs.

Participates in the admission team responsibility of providing seven (7) days week coverage; including evenings and holidays.

Recognizes, reinforces and enhances the role of the volunteer in Hospice services provided.

Visits referral sources to educate about Hospice services.

Educates the c! ommunity regarding hospice philosophy and concept through a case by cas! e consultation with person in the community or educational in-service or seminar.

Participates in orientation from initial classroom to field integration.

Supports Marketing Representatives with referral sources and activities.

Represents Catholic Hospice at nursing functions in the community.

Participate with on-call rotations.

Performs other duties as assigned by supervisor.

LANGUAGE SKILLS:
Ability to read, analyze, and interpret common information, reports and medical documents. Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the Healthcare industry. Ability to effectively present information to management, public groups, referral sources or patients and caregivers. Ability to speak and write Spanish is strongly preferred.

The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. Field positions will be exposed to various unfamiliar conditions which may include, but are not limited to: patient homes, nursing homes, assisted living facilities, doctor's offices and hospitals. MINIMUM QUALIFICATIONS:
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Current Florida License as a Registered Nurse.

BSN, Graduate Diploma or Associate Degree Program graduate.

Minimum of two (2) years nursing experience. Hospice Nursing experience preferred.

Excellent oral and written communications skills in English. Bilingual (English / Spanish) strongly preferred

Proficiency in word processing and general computer applications, including, but not limited to Microsoft Office. Experience in Suncoast or Medicare / Medicaid systems a strong preference

Must be 18 years or older

Pass the! drug test

Ability and willingness to work a flexible Full Time schedule that includes weekends, evenings and holidays

Must be able to use facsimile, write and transcribe information accurately

Must be able to provide own transportation to various locations in service areas, as required by duties
