Case Manager

Bachelor's degree in Social Work preferred, will consider BA or BS degree in other related field. Also requires knowledge of community services. Experience in a mental health setting preferred. Strong communication, teaming and time management skills necessary.

Essential Job Functions
Accurately performs screening/intake procedures
Documents status and progress and provid! es education to clients/family. Acts as liaison between client! , doctor and family
Provides basic case management services to clients
Assists in treatment planning and review
Finalizes discharge plans
Other duties as assigned or required Schedule: Monday - Friday 8:30am - 5:00pm
What is the Med Clinic:
Med clinic is a fast paced program that serves more than 3000 active patients. The program staffing includes 6 doctors, 4 ARNP's, 3 nurses, 5 care coordinators, and 4 support staff. On most days, there are between 80 and 120 clients who attend med clinic appointments, and nearly 100 phone calls for issues ranging from prescription refill requests to Medication side effects. The goal of the med clinic is to support the physician services at Manatee Glens West. Care coordinators do the state paperwork and perform limited case management functions to the med clinic clients. Nurses support the doctor services by triaging client issues. Support staff provide limited clinical inter! actions including vital signs and medication reconciliations for clients at their appointment. All Med Clinic staff have extensive face to face contact with our clients, and must have good customer service skills to deal with clients.
