For detail informations about this job opportunity please read the description below. Halifax Health is seeking an LPN for Halifax Health-Hospice of Volusia/Flagler's Care Centers . The Licensed Practical Nurse will provide direct patient care to assigned patients under the direction and supervision of a Registered Nurse. Job responsibilities include carrying out selected procedures, administration of medications, and treatments requiring understanding and technical ability in accordance with Halifax Medical Center 's policies for job related education and experience.

Requirements:
Currently licensed or eligible for LPN licensure in State of Florida . Graduation from an approved School of Nursing in a course of training for Licensed Practical Nurses. Must have one year's experience in Med-surg or other acute care area. Valid driving license and good driving record are required for this position. Professionalism in interpersonal communication skills with patients, patient families, colleagues, physicians and ancillary department personnel. Certification from a 30 hour IV course preferred. Additional certification provided through the facility for IV responsibilities after certification recieved.

Responsibilities:
Provides compassionate care to the terminally ill and their families.

P erform and document complete assessments.

Reports any change to supervising RN.

Provides patient and family education.

Reinforces the implications of the diagnosis/prognosis and the nature of treatment based on information from primary nurse and/or physician.

Works in collaboration with an Interdisciplinary Team to develop a Plan of Care which reflect pt./family centered goals.

Uses critical thinking skills.

Functions as a representative of HOVF and utilizes effective communication methods and skills.

Participates in quality improvement activities.

Maintains professional and positive working relationships.

Promotes a holistic approach to the care of the terminally ill
