General Summary

Under the general supervision of the Director Case Management, t he CDS is responsible for reviewing medical records to facilitate the accurate representation of the severity of illness. This involves extensive record review, interaction with physicians, HIM professionals, and nursing staff. Active participation in team meetings and education of staff in the CDMP process is a key role.

RESPONSIBILITIES

1. Ability to learn/develop the skills necessary to perform CDMP

2. Accurate and timely record review

3. .Recognize opportunities for documentation improvement

4. Formulate clinically credible documentation clarifications

5. Effective and appropriate communication with physicians

6. Timely follow up on all cases especially those with clinical documentation clarifications
7. Participate in Task Force Meetings

8. Manage multiple priorities

9. Communicates with HIM staff and resolves discrepancies

10. Accurate input of data into CDMP Trak

11. Reports observed deviations from established policies and procedures and standards of care to the immediate supervisor in a confidential manner. Protects the patient’s rights; maintains confidentiality of all patient related information.

12. Intervenes when variances occur in the patient’s individualized discharge plan.

13. Demonstrates the team concept; works in a cooperative manner with co-workers and supervisory staff.

14. Utilizes the appropriate chain of command to resolve conflicts.

15. Assists with orientation of new employees and provides learning experience for affiliate students.

16. Contributes to the smooth operation of the Clinical Case Management Department by performing other related duties as assigned ! by the Director of Case Management such as but not limited to:

! 17. Performs other tasks as assigned which are within the employee’s capability.

Skills Required:
Organizational, analytical, writing and interpersonal skills

Dependable, self-directed and pleasant

Critical thinking, problem solving and deductive reasoning skills

Recent hospital Experience preferably ICU, CCU, or strong Med/Surg.

G eneral understanding of hospital based Quality initiatives (training provided)

Understand and support CDMP® documentation strategies (upon

completion of training)

Understand and communicate differences between Medicare Part A

and Part B guidelines and how they impact DRG assignments

Experience

Graduation from a program of nursing, BSN, preferred.

Current state Registered Nurse license, Preferred â€" a CD Certification / Accreditation

Knowledge

Knowledge of Pathophysiology and Disease Process

Knowled! ge of Medicare Part A

Familiar with Medicare Part B

Knowledge of regulatory environment

Knowledge of InterQual, Milleman and Robertson Criteria, Principles of Managed Care, Computer applications, including but not limited to: Windows, Outlook, Excel

and Microsoft.

Word ing knowledge of Cerner is an asset.
