Location: North Sarasota Florida

Description: Tidewell Hospice is currently seeking to employ LPN â€" Triage right now, this occupation will be placed in Florida. Detailed specification about this occupation opportunity please read the description below. Statement of Purpose:
Under physician’s orders and RN supervision, the Licensed Practical Nurse (LPN) is responsible for responding to afterhours (night time and weekend) patient ! needs. The LPN is responsible for providing clinical, palliative and emotional support to the patient/family unit within the scope of practice.

Primary tasks, duties, and responsibilities:
1.Promotes and practices the hospice mission and values, and follows all policies and procedures of Tidewell Hospice.
2.Visits patients and provides clinical, physical, educational and emotional support after-hours as directed by the RN/Triage team, reporting significant changes in patient condition as needed.
3.The reasons for the visit will include, but are not restricted to: wound care, vital sign observations, blood sugar monitoring, intramuscular and subcutaneous injections, medication administration, IV infusion therapy and Foley catheter insertion.
4.Attends patient deaths, provides bereavement support to the patient caregiver/ family, as indicated and performs post mortem care.
5.Demonstrates clinical skills regarding application of Tidewell Hos! pice clinical and administrative policy and procedures such as! pain medication titration under the direction of an RN and physician.
6.Utilizes nursing interventions and teaching skills to maximize patient comfort and to enhance the quality of life to the patient/family unit.
7.Provides accurate and timely documentation of patient services within electronic medical record to reflect the individualized plan of care and outcomes of interventions.
8.In collaboration with the RN, accepts and processes physician orders to maximize patient comfort.
9.Accepts responsibility and is accountable for nursing actions; requests RN supervision when needed; performs within the guidelines established by the Florida Nurse Practice Act.

Secondary Responsibilities:
Every incidental duty connected with the LPN Triage position cannot be specified in the job description and the colleague, at the discretion of the Clinical Director, may be required to perform duties that are not included in this job description.

! Physical Requirements:
1.Heavy work; frequently uses 25-50 pounds of force to move objects and occassionally uses proper techniques/assistance to lift greater weights, including the human body.
2.Ability to turn, reposition, and assist patients to transfer and ambulate.
3.Auditory and visual acuity, (including near/far vision) color discrimination and depth perception; ability to use all senses necessary for patient observation, communication and driving.
4.Sitting, standing, walking and driving for prolonged periods of time.
5.Frequently stoop, kneel, and crouch during the provision of patient care.
6.Fine motor skills to reach to administer patient care services, treatments and procedures and grasp objects, including medical supplies/equipment, the human body and a writing instrument, during the provision of patient care.
7.May ascend and descend stairs, uneven surfaces, or obstacles, which may be located indoors or outdoors, and may not! be handicap accessible.

Working Conditions:
1.Subj! ect to a variety of physical conditions within individual patient homes; safety issues may include basic home safety, environmental safety, and bathroom safety.
2.Subject to occasional exposure to hazards associated with blood, potentially infectious materials and hazardous waste.

Educational/Professional:
1.Current and active Florida LPN license.
2.Completion of an approved 30 hour IV certification course required.
3.One year of previous acute health care experience required.
4.Previous Hospice/Home Health Care experience required.

Knowledge, Skills and Abilities Required:
1.Possess strong patient observation management and documentation skills.
2.Able to work flexible hours (evenings, weekends, holidays) as required to meet identified patient/ family/ organizational needs.
3.Able to respond effectively to meet the treatment needs of chronically, acutely and terminally ill clients and their caregivers.
4.A! ble to adapt to changing organizational needs.
5.Speak, write, read, and comprehend English. Bilingual (English/Spanish) a plus.
6.Excellent organizational, interpersonal, telephone and communication skills.
7.Computer proficiency and keyboarding skills of 30wpm.
8.Valid State of Florida driver’s license and automobile insurance.
9.Ability to provide own transportation and travel throughout service area.

Machines/Equipment, Tools Used:
1.Uses supplies, equipment, medication, and patient therapies as per Tidewell Hospice policies.
2.Must be able to use fax, telephone and computer.
