The Visiting Nurse Association of Florida is currently seeking a Registered Nurse.
SUMMARY
Our nurses ensure the delivery of the highest quality nursing care services for patients through comprehensive assessments, compliance with current physician's orders, appropriate interventions, reports to the physician, and complete and accurate documentation in the clinical record.
ESSENTIAL DUTIES AN! D RESPONSIBILITIES
Provides quality and safe delivery! of home health care services by implementing current standards of nursing practice. Demonstrates competency in necessary clinical skills.
Evaluates and admits patients to service after conducting a comprehensive assessment (including the OASIS data), taking a complete nursing history, and evaluating the home situation to determine if the case is appropriate for home care. Initiates appropriate clinical interventions to meet the goals of care. Consults with Supervisor and the physician as appropriate.
Conducts OASIS assessments accurately, according to instructions in Chapter 8 of the OASIS Implementation Manual, and corresponding to documentation contained elsewhere in the assessment note.
Understands the home health coverage guidelines for Medicare/Insurance and determines whether the care required meets those guidelines. Advises the patient of the coverage, of any potential out of pocket costs, and of options available. Presents and explains th! e Medicare Advance Beneficiary Notice form to Medicare patients when necessary.
Is responsible for establishing a plan of care and clinical pathway based on the physician's orders, with periodic revisions as necessary, and following those physicians orders. Is responsible for reducing any Verbal Order to writing (the day it is given) to be signed by the physician.
Develops reasonable, specific and measurable goals that commensurate with the plan of care, are agreed upon by the patient/care giver. Counsels and instructs in order to accomplish goals.
Assesses the patient's condition and reports significant changes to the physician, supervisor and other team members. Assures that written progress reports are made to the physicians as needed.
Initiates discharge planning with patient/care giver at time of admission and throughout the case.
Prepares home health aide assignments and guidance to the aides. Performs supervisory visits at! least every 14 days to update the Aide assignment and evaluate the eff! ectiveness of the care.
Complies with agency regulations regarding documentation and maintains clinical record for each patient. Will attain necessary proficiency in use of agency computer system for clinical documentation. Will communicate clinical notes/contacts as prescribed by the agency, however no later than the end of the day of service.
Attends Team Conferences and participates in discussions for specific problem solving measures for patients in case load.
Consults with Supervisor whenever goals are not being met, cases fall outside expected parameters.
Available to work at least 2 weekend per month.
Full- time staff only - Maintains a productivity standard of at least 33 units per week (66 per pay period).
Fulfills additional performance responsibilities as assigned and as necessary.

Graduate of an approved school of nursing and is currently licensed in Florida. One year of hospital experience in the field of Med/Surg nursing is preferred.
Nurses who perform Infusion or Hi-Tech services must have additional training/experience and have demonstrated proficiency in these procedures.
Nurses who provide Psychiatric services must have been approved by the Medicare intermediary prior to providing these services.
Nurses who proved specialized wound or ostomy consultation services to agency staff must have additional training from a WOCN accredi! ted program
