Location: North Sarasota Florida

Uses the nursing process and nurse case management strategies to coordinate care and provide nursing services for hospice patients and their families in the home, long term care facility, assisted living facility, hospice house or hospital.

Primary tasks, duties, and responsibilities:
1.Promotes and practices the Hospice Mission and core values approaching patient/family/caregivers with compassion and respect.
2.Is an effective team member who uses communication and leadership strategies, time management, effective problem solving and an awareness of ethical and cultural issues while providing ca! se management for assigned patients/families.
3.Is aware o! f and follows the Florida Nurse Practice Act, Standards of Practice of Hospice Nursing as well as legal, regulatory and CHAP requirements.
4.Provides initial and ongoing assessment of the impact of the terminal diagnosis on the patient’s physical, functional, psychosocial and environmental needs and activities of daily living including: (a) risk for pathological grief, (b) cultural and spiritual implications, (c) verbal and non-verbal communication patterns.
5.In collaboration with the other members of the IDT, develops and implements the individualized plan of care and makes and recommends revisions to the plan as necessary.
6.Under the direction of a licensed physician, manages discomfort and provides symptom relief using specialized nursing skills related to palliative and end-of-life care.
7.Consults with and educates the patient/family regarding: (a) safety, (b) the disease process, (c) self-care techniques, (d) end-of-life care, (e) processes ! for dealing with issues of ethical concern.
8.Initiates appropriate preventive and rehabilitative nursing procedures.
9.Prepares clinical and progress notes that demonstrate progress toward establishing goals.
10.Coordinates all patient/family services and prioritization of needs with the members of the interdisciplinary team.
11.Uses case management approach and strategies and makes referrals to other services as needed.
12.Establishes and maintains effective relationships with physicians, informing the physician and other personnel of changes in the patient’s needs and outcomes of intervention.
13.Guides the IDT to determine scope and frequency of services needed based on acuity and patient/family needs.
14.Supervises LPN’s and paraprofessionals providing services to the patient according to regulations.
15.Continues to develop competence and skill through participation in in-service programs, awareness of developments in hospice nursin! g and nursing practices in general and interest in personal development! .
16..Provides specialized hospice and personal care training to other staff, family members and informal caregivers to insure adequate care.
17.Integrates on-going evaluation of patient/ family response to care in planning, implementation and documentation.
18.Assesses the ability of the caregiver to meet the patient’s immediate needs upon admission and throughout care, providing education, support and interventions as appropriate.
19.Evaluates own needs for support and uses identified resources to meet the need. Is able to recognize and maintain professional boundaries in relationships with patient/family/caregiver.
20.Applies specific criteria for admission and re-certification to hospice care to establish appropriate levels of care and patient’s eligibility.
21.Promotes efficient and quality care by adhering to productivity standards and active participation in Performance Improvement activities.
22.Develops and maintains working knowledg! e of clinical software system uses point of service documentation whenever possible and communicates effectively using current processes and available technology.
23.Reviews and updates PIO’s/ Careplan, physician’s orders, MAR/ Pharmacy profile, PPOC and other records to ensure patient/ family needs are met.
24.Collaboratively plans and communicates with members of facilities to ensure quality care and effective relationships.
25.Is professional in all interactions and behaviors including adherence to dress code as per Tidewell Colleague Handbook.
26.Is familiar with policies and procedures and follows these in all work related activities.
27.Promotes and practices safety including compliance with traffic regulations, safe driving and wearing a seat belt when business travel is required.
28.Is available throughout disaster for specific assignments by supervisor including staffing special needs shelters.
29.All other duties as assigned.
Physical Requirements:
1.Visual and auditory acuity.
2! .Able to lift.
3.Able to drive during daylight and night time.
4.Good communication and collaborative skills.

Educational/Professional:
Graduate of approved nursing program â€" RN current Florida license.

Experience:
1.Minimum of one (1) year clinical hospital experience.
2.Previous experience in hospice or home health preferred.
3.CHPN certification is strongly encouraged within 3 years of employment.

Knowledge, Skills and Abilities Required:
1.Self-starter, dependable, little need of supervision, current clinical skills, flexible and tactful.
2.Possess a valid Florida driver’s license.
3.Possess good clinical, counseling and organizational skills.
4.Demonstrated knowledge and experience in working with patients, families dealing with life threatening illness and death, federal, state, HIPPA, CHAP and/or other regulations as needed.
5.Is able to demonstrate an effective and workin! g knowledge of computers, clinical information systems, software related to pharmaceutical information/ management and communications.

Machines/Equipment, Tools Used:
1.Equipment utilized in a clinical practice
2.Fax, telephone and computer/printer.
3.Is able to type and use a computer keyboard with ease.
