Humana is seeking Nurse Practitioners for general medical quality care and treatment to patients within our clinical facilities with the ability to consult with the medical staff:
Provides general medical care and treatment to patients in medical clinic under direction of Physician.
Performs physical examinations and preventive health measures within prescribed guidelines and instructions of Physician.
Orders, interprets, and evaluates diagnostic tests to i! dentify and assess patients’ clinical problems and health care needs.
Records physical findings, and formulates plan and prognosis, based on patient’s condition. Provides written home-going instructions.
Collaborates with Physician and other health professionals to prepare comprehensive patient care plan as necessary.
Prescribes or recommends medications or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures.
Refers patients to Primary Care Physician for consultation or to specialized health resources for treatment.
Educates and coaches nursing staff on best nursing practices.

Qualifications
Role Essentials
Bachelors of Science in Nursing (BSN) from an accredited program.
Masters of Science in Nursing (MSN)
Successful completion of a Nurse Practitioner program
Active, unrestricted Registered Nurse and Nurse Practitioner license in the state indicated as the “Location†above.
Current certification or able to be certified in AHA or ARC Basic Life Support for medical provider.
Appropriate certification to write prescriptions under the ! authority of a medical director.
Computer proficiency with kno! wledge of MS Office products, specifically Word, Excel, and Outlook.
Experience with electronic health record documentation.
Role Desirables
Certification in Adult Health, Family, Acute Care or Gerontology preferred.
Minimum of two years post-training clinical experience beneficial.
Experience in disease management and wellness programs a plus.
Previous Medicare/Medicaid Experience a plus.
Knowledge or experience with a patient centered medical home model.
Experience with eClinicalWorks is beneficial.
Bilingual: English, Spanish and other languages are beneficial.
