.We are looking for REGISTERED NURSES to become members of the interdisciplinary team who is the pivotal person responsible for identifying the physical, psychological, social, and spiritual needs of those patients and families needing assistance after normal business hours. He/She initiates appropriate interventions and support for the patient and family during the on call hours.
Job Duties
Provides clinical assessment and intervention via the telephone utilizing the nursing pro! cess.
Serves as the patient and family advocate by communi! cating with the attending physicians, long term care facility staff, case managers, and others external to Client as necessary.
Coordinates all service delivery during the on call hours including the education, dispatching of interdisciplinary visits, and coordination of services with external vendors and resources.
Provides bereavement support to families.
Documents all interactions with patients, families, contacts, and vendors in the computer record or appropriate back up system and demonstrates the collaborative process in their documentation.
Clearly documents the assessment, plan of care, any actions and interventions and the resolution of each patient/family interaction.
Reviews all entries in the record made by triage support staff to ensure professionalism and accuracy.
Provides skilled nursing as determined by the interdisciplinary plan of care according to protocols.
Educates patients, families, and other care persons in appropriate ca! re techniques by telephone intervention.
Assesses patient and family care to ensure the appropriate level of service delivery occurs as indicated by patient symptoms and caregiver needs.
- Participates in the quality assessment and improvement activities as assigned by the supervising manager.
- Participates in proactive patient calls.
- Participates in the reporting of incidents and infection control surveillance responsibilities.
- Monitors employee productivity as directed by the supervising manager to ensure patient needs are as met as expeditiously as possible.
- Basic computer skills and typing
- Ability to sit for lengthy periods
- Currently licensed to practice in Florida and eligible for licensure in other states
- A minimum of two years experience in acute-care hospital nursing in either medical-surgical, oncology, or emergency.
- Ke! yboarding and basic Computer skills
- Customer service knowledge! and good phone skills
- Good phone voice and communication skills
- Ability to work as a team player
