Role: The Bereavement Specialist (BVS) provides a range of bereavement services to bereaved survivors. Qualifications:
Bachelors degree or above in Social Work (CSWE accredited) or related degree.
Experience in health care/hospital setting preferred.
Familiarity with and working knowledge of the concepts and practices of death and dying with children and adults.
Ability to remain calm and professional and to trouble shoot and manage patient/family crisis situations.
Valid driver's license and automobile ! insurance per HPC policy and procedure.
Ability to tr! avel to off-site locations.
Ability to work unsupervised, exercising a high degree of discretion & independent judgment.
Ability to physically access any home, hospital, nursing home, and ALF, i.e. navigate stairs, narrow stairs, narrow spaces, lack of air conditioning, etc.
Responsibilities: Administration Participate in interdisciplinary group (IDG) meetings as needed; providing feedback about patients and family members. Attend staff and department meetings, in-service training and continuing education workshops as needed. ⢠Maintain bereavement documentation in a manner reflecting timeliness, completeness, accuracy, legibility, clarity of goal, evaluation, plan, and intervention. ⢠Participate in Quality Assessment and Performance Improvement activities as directed by the Grief Services Coordinator/Social Services Manager. ⢠Responsible for Volunteer training (Social Services/Bereavement) as assigned. ⢠Participate in HPC commit! tees as assigned. ⢠Maintain focus on care planning needs and flexibility with changing survivor needs.
Responsibilities: Caseload ⢠Evaluate the bereavement care needs for survivors (i.e. adults, children, families) in accordance with HPC policy. ⢠Provides grief guidance and support to maintain the Plan of Care ⢠Meet established productivity expectations. ⢠Experience in children, family, and group modalities. ⢠Facilitate bereavement and holiday support groups for children and adults throughout the county and the year. ⢠Participate in on-call assignments, providing 24 hour, seven day a week patient and family care; responding to bereavement care needs. ⢠Maintain bereavement documentation in manner reflecting timeliness, completeness, accuracy, legibility, clarity of goal, evaluation and plan, and appropriateness and relevance of intervention. ⢠Participate in and support development of Bereavement Programs: Groups, Camps, Memorial Ser! vices, etc. ⢠Participates in on-call assignments IDG Participation â! ¢ Attend/participate in interdisciplinary group meetings as needed; assists in developing Plan of Care focus; facilitates understanding of bereavement aspects of care; enables team to develop Plan of Care goals. ⢠Consult/collaborate with other professionals to promote quality and continuity of care. ⢠Demonstrate respect for fellow professionals and IDG leadership. Professional Development ⢠Enhance professional expertise and knowledge. ⢠Participate in appropriate professional or specialty organizations. ⢠Demonstrate creative critical thinking in problem solving patient/family issues. Other ⢠Perform other duties as requested by the Grief Services Coordinator/Social Services Manager. ⢠Meet or exceed HPCâs standards in all areas. ⢠Incorporate behaviors supportive of internal mission statement. ⢠Promote the hospice concept in the community by participating in the development of educational materials and offerings for both the public and medical comm! unity. ⢠Works within HPC Scope of Service
